Montana Med Spa and Wellness

1. Acceptance of Terms
By scheduling an appointment, receiving services, or purchasing products from Montana Med Spa and
Wellness ("we," "our," or "us"), you agree to the following Terms of Service. If you do not agree, please
refrain from using our services.

2. Eligibility
Services are available only to individuals aged 18 or older. Clients under 18 may receive certain treatments
with written parental/guardian consent and in accordance with Montana state law.

3. Medical Disclaimer
All treatments provided are non-surgical aesthetic and wellness services, not intended to diagnose, treat,
cure, or prevent any disease. Our services do not replace professional medical advice. Clients should consult
with a qualified healthcare provider regarding any medical concerns before booking.

4. Consultations & Informed Consent
A consultation is required prior to any treatment to ensure it is appropriate and safe for you. You must
complete a health history form and disclose all relevant medical conditions, allergies, and medications.
Failure to disclose accurate information may result in cancellation of services and could impact your results
and safety.

5. Appointment Booking & Cancellation Policy
- A valid credit card may be required to reserve your appointment.
- Cancellations or rescheduling require at least 24 hours' notice.
- Late cancellations or no-shows may result in a fee of $100.
- If you are more than 10 minutes late, we may shorten or reschedule your appointment.

6. Treatment Results & Guarantees
Individual results vary and cannot be guaranteed. Multiple sessions may be necessary to achieve desired
outcomes. You are responsible for following pre- and post-care instructions provided by our team.

7. Payments & Refund Policy

- Payment is due in full at the time of service.
- We accept all major payment methods for your convenience, including cash, credit/
debit cards, checks, and mobile payments such as apple/google pay.
- All treatment and package sales are final and non-refundable.

8. Client Responsibilities
You agree to:
- Follow all pre- and post-treatment instructions.
- Inform us immediately of any adverse reaction or unexpected change in your health.
- Refrain from disruptive, inappropriate, or unsafe behavior.

9. Contraindications
Certain medical conditions, medications, or lifestyle factors may make you ineligible for some treatments. Our
providers reserve the right to refuse service if a treatment is not safe or suitable for you.

10. Photography & Media Release
We may take before-and-after photographs for treatment planning, medical records, and/or marketing
purposes. Photos used for marketing will not include identifying information without your written consent.

11. Treatment-Specific Waivers
In addition to this Terms of Service, each treatment may require you to sign a separate consent form
acknowledging potential risks, benefits, and alternatives. These may include but are not limited to:
- Neurotoxin Injections (Botox, Dysport, Xeomin, Jeuveau): Possible side effects include temporary bruising,
swelling, asymmetry, headaches, or drooping eyelids/eyebrows.
- Dermal Fillers: Possible side effects include swelling, bruising, lumps, asymmetry, infection, or vascular
occlusion (rare but serious).
- Microneedling / Chemical Peels / Skin Resurfacing: Possible side effects include redness, peeling,
hyperpigmentation, or infection.
- IV Infusions / Vitamin Injections: Possible side effects include bruising at injection site, vein irritation, allergic
reaction, or electrolyte imbalance.
- Weight Loss Injections (Semaglutide, Tirzepatide, etc.): Possible side effects include nausea, vomiting,
diarrhea, constipation, and potential contraindications with certain medical conditions.
- PRP / PRF / EZ Gel Treatments: Possible side effects include redness, swelling, bruising, tenderness, or
infection.

To the maximum extent permitted by law, Montana Med Spa and Wellness and its staff are not liable for any
indirect, incidental, or consequential damages resulting from the use of our services. You agree to hold us
harmless for any claims related to your treatment except in cases of proven gross negligence or willful
misconduct.

13. Privacy & Confidentiality
Your personal information and treatment records are kept confidential in compliance with HIPAA and
applicable laws. We will not share your information without your consent except as required by law.

14. Governing Law
These Terms of Service are governed by the laws of the State of Montana. Any disputes shall be resolved in
the courts of [Insert County], Montana.
